The Center for Housing and Health (CHH) is a Chicago-based nonprofit subsidiary of AIDS Foundation Chicago. It works at the junction of housing systems and healthcare services, with the aim of improving outcomes for Chicagoans experiencing homelessness. Its technical domains include housing systems coordination, integration of healthcare services, case management, health navigation and the delivery of supportive services.
CHH administers the Flexible Housing Pool of Chicago and Cook County (FHP), a multi-agency programme that helps people experiencing homelessness obtain stable housing and a package of supportive services. These include case management, health navigation and links to medical care, benefits and community resources. The pool coordinates with more than 28 supportive housing agencies across Chicago and Cook County, which together form the organisation's principal service footprint.
The organisation follows a Housing First philosophy: it links people to permanent housing quickly, with minimal barriers and no preconditions such as sobriety or treatment compliance. Service delivery is trauma-informed and client-centred, tailored to each individual's circumstances while pursuing housing stability and better health outcomes. CHH sits within the nonprofit, housing and homelessness services, healthcare and social services sectors.
