Alivia Care

Alivia Care, Inc. is a 501(c)(3) nonprofit organisation founded in 2020 and headquartered in Jacksonville, Florida, with operations across North Florida and South Georgia. It provides leadership to support care solutions for patients with advancing illness through services, collaborative partnerships, and strategic investments. The organisation emerged from a multi-year strategic planning effort by the board and executive leadership of Community Hospice & Palliative Care. Alivia Care operates a family of three programmes. Community Hospice & Palliative Care delivers hospice and palliative care in homes, inpatient centres, nursing facilities, and hospitals. The PACE Place is a Program of All-Inclusive Care for the Elderly. Alivia Care @ Home provides skilled nursing, therapy, and supportive care in patients' homes. Work spans hospice care, palliative care, PACE, home health and skilled nursing, therapy and supportive in-house care, and coordinated, person-centred care delivery. The organisation's stated orientation is toward person-centred care for patients living with advancing illness, and its stated vision is to expand access to such care beyond traditional hospice services. Alivia Care works through collaborative partnerships with hospitals, physicians, health plans, and community organisations, alongside strategic investment in care solutions.
