Community Care Plan (CCP) is a Florida-headquartered health plan organisation founded in 2000, originally known as South Florida Community Care Network. It is Florida's first and longest-standing Provider Service Network (PSN) and serves more than 120,000 members. The company is owned jointly by two public healthcare systems: Broward Health (North Broward Hospital District) and Memorial Healthcare System (South Broward Hospital District).
CCP's coverage spans several lines of business. Its Medicaid (MMA) managed medical assistance plans are available across 19 Florida counties. Florida Healthy Kids plans cover children aged 5-18 in 8 counties, offered in partnership with Florida Healthy Kids. The organisation also provides self-insured employee health plans for employer groups, runs ACA marketplace plans for Broward County residents through its 22 Health brand, and manages uninsured programmes and value-based care initiatives through its SydCura division.
